Services offered:
Skilled Nursing is provided by experienced RN or LPN. The nurse communicates frequently with your physician to update your plan of care, coordinate services to your physician and other health care team members. Physical Therapists work with you to relieve pain, build your strength; improve your balance and coordination Occupational Therapists help you to promote independence in your personal care, reduce your physical limitations, learn coping skills; and improve ability to perform daily living tasks. Speech Therapists work to find the cause of any speech problems you may have, and teach you how to improve your speech and language skills, swallowing, problem solving and general skills Medical Social Services are provided by Medical Social Worker, Services may include short-term counseling services, referral to and coordination with community resources and assistance with living arrangements, emotional, social and financial problems and long-range planning. Home Health Aide services are delivered under the supervision of a registered nurse or licensed therapist. Our aides have had experience caring for people in the home. Our aides will help you care for yourself (bathing, dressing, changing bed linens and assistance with activities of daily living, etc.); prepare light meals and provide light cleaning to the immediate environment to keep it safe and orderly. Home Health aides also alert your home care nurse to any changes in your health. Supplies/Therapy: Medical supplies and therapy services may be required to carry our your plan of care. All medically necessary therapy services or medical supplies must be coordinated with the home health agency while you are receiving Medicare covered home health services. If you arrange for these services or supplies on your own while under our plan of care, Medicare will not reimburse you or the other suppliers.
